Early Pregnancy tests and Ovulation

Pregnancy tests come in a range of sensitivities, as a general rule, the lower the sensitivity, the earlier the test will give a positive result. Recent research has shown that in up to 10 percent of pregnancies, implantation is delayed, and testing may not become positive until as late as day 12 after the missed period.

We know lots of people wish to test early, and
for some, with irregular or erratic cycles, knowing when to test can be
challenging. In this situation, the earliest time at which hCG
is possibly detectable is 8 –10 days after conception. This is dependent on
early implantation and placental development to a level able to cause urinary
concentrations of hCG in excess of the sensitivity of
the pregnancy tests used. If you do decide to test early, always use your first morning
sample, and be prepared to re-test at 48 hr intervals, and if you do get very
early positive results, always confirm these a few days later (as some very
early implantations do not continue) Our advice if your cycle is predictable,
is to wait until your period is late before testing.
Ovulation tests are a great way to pin point
the most fertile period in each cycle. By doing so the chances of conception in
any given cycle can be maximised by timing intercourse around ovulation.
Ovulation tests work by detecting the surge in LH (Luteinising
Hormone) which occurs to trigger the release of a ripe egg from an ovary.
Ovulation normally occurs within 24-36 hours of the LH surge. The egg is then
viable for 24-36 hrs, beyond this time, fertilisation
is unlikely to occur until the next cycle. By detecting the LH surge, couples
are able to determine the most fertile 24 - 36 hours
of each menstrual cycle. Timing sexual intercourse,
within this window can help maximize the chances of successful conception.

Ovulation tests can be useful
for anyone who has been trying to conceive for a while unsuccessfully and needs
to know if they are ovulating regularly. If no surge in LH is detected over 3
months this will help direct your doctor when considering further investigations
for infertility.

If the first pregnancy test is negative and
the symptoms of pregnancy continue, the pregnancy tests should be repeated at
24-48 hrs intervals using a first morning urine
sample. (see paragraph two.)
If your home pregnancy test is positive you should
arrange to see your doctor to discuss the pregnancy. Depending on the policy of
the health care centre, they may wish to confirm the result, so take a sample
of urine if possible.
Beyond day 12 from the missed period, if home
pregnancy testing remains negative, but the symptoms of pregnancy continue, you
should see your doctor to discuss laboratory pregnancy tests or blood
tests.
We do not recommend or suggest testing before
the day of the missed period. Testing too early can give false negative and
false positive results. This is because the levels of hCG
in the urine sample being tested may be below the cut off sensitivities of the
test despite a viable and developing early pregnancy. Also, not all early
implantations proceed; some studies have estimated that up to 20% of very early
pregnancies fail to proceed. As the hCG levels are most concentrated in urine
produced overnight, it is recommended that early pregnancy testing is performed on a urine sample
produced first thing in the morning. However in the majority of
pregnancies (>90%) by the time of the missed period, testing will be positive
at any time of day. (see paragraph two relating to
delayed or late implantation)
All
on site pregnancy tests can give false positive results in the
weeks after delivery (as it takes time for the levels of hCG to return to
normal after pregnancy, normally 4 weeks) This is also the case after a
miscarriage or termination. hCG can also, rarely be
produced by some tumours. hCG is produced in ectopic pregnancy, and on-site pregnancy tests will not differentiate these
conditions.
The pregnancy and ovulation tests are
not effected by Clomid.
After fertility treatment with hCG constituents, the pregnancy tests should not be used for
14 days
